Litchfield Family Practice Center

Text messaging terms and conditions

Effective date:

JardogsHealth hosts this page on behalf of Litchfield Family Practice Center, LLP, which provides its patient text messaging through the JardogsHealth platform.

Program description

Litchfield Family Practice Center, LLP sends text messages, under the program name “Litchfield Family Practice Center Patient Texts,” to its established patients at the mobile number they give the practice. Patients who choose No at check-in or reply STOP do not receive them. Messages include appointment reminders and confirmations, notices that a new statement or an update from your care team is available in your patient portal, requests for information we need before your visit (such as a copy of your insurance card), and replies from our staff during office hours. We do not send marketing or promotional messages.

How you opt in

You give us your mobile number when you register as a patient. When you check in for a visit, on your check-in link or on the tablet in our office, we ask: “May we send you text messages about your appointments and care?” You choose Yes or No. If you choose No, we will not send you text messages, including appointment reminders and check-in links. Agreeing to text messages is optional and is not a condition of receiving care.

See how patients agree to text messages for the check-in screen.

Message frequency and cost

Message frequency varies. Most patients receive a reminder before each appointment and occasional notices. Message and data rates may apply.

How to opt out

Reply STOP to any message to stop receiving text messages from us. You can also reply STOPALL, UNSUBSCRIBE, CANCEL, END, or QUIT. We will send one final message confirming that you are unsubscribed: “You have been unsubscribed from Litchfield Family Practice Center text messages and will receive no more texts from this number. Reply START to resubscribe.” To receive messages again, reply START.

Help

Reply HELP to any message, or call us at 217-324-6127. Our reply: “Litchfield Family Practice Center: for help call 217-324-6127. Reply STOP to unsubscribe. Msg & data rates may apply.”

Emergencies

Do not use text messages for medical emergencies. If you have an emergency, call 911.

Carriers

Carriers are not liable for delayed or undelivered messages. Text messaging is available on most major U.S. mobile carriers.

Privacy

Our text messaging privacy policy explains how we handle your mobile number and messages. We do not sell, rent, or share your mobile phone number or text messaging opt-in data with third parties or affiliates for marketing or promotional purposes.

Changes to these terms

If we change these terms, we will post the updated version on this page with a new effective date.
